Your signature releasing your transcript on: i. A letter requesting the transcript (signed) ii. A completed request form in the Registar’s Office iii. A FAX letter requesting a transcript (signed) 2. Include your Student ID# or SSN and either an e-mail address of phone number where you can be reached during the day. 3. Include the name of the Person and/or Office and Institution, and mailing address of the recipient.
