Requirements:
Shenandoah University
requires
that every student have a completed
health form on
file at the Wilkins Wellness Center.
All Shenandoah
University students are required to provide proof of health
insurance coverage (copy of insurance card) to the Wilkins Wellness Center prior to
registration.
Appointments and Evaluations:
Students must be triaged
by the nurse at the Wilkins Wellness Center to schedule an appointment with the Certified Family
Nurse Practitioner.
Class Excuses: Notes from the wellness center
are for documentation purposes only, they do not excuse
the student from classes.
Family Death:
The Wilkins Wellness Center will email professors when a student is
out due to the death of a family member. Please contact the
wellness center for additional requirements at (540) 665-4530.
Eligibility: All SU students
are eligible for services provided by the Wilkins Wellness Center.
Billing and Insurance:
Visits to the Wilkins Wellness Center for evaluation are free of
charge, however some services are not. The wellness center
does not file insurance claims. Any charges incurred will be
billed to the student's account.
Information regarding charges is confidential and
will only be released to the student.
Medical Records: The Wilkins Wellness Center does not accept
requests for medical records through the internet. For legal
reasons, you must provide a signed
HIPAA form. This request
may be faxed or mailed.
Holds on
Registration:
If you have a hold on your
registration from the Wilkins Wellness Center, it is probably due to
an incomplete health form.
In almost all cases, our office has
notified you of the deficiencies via your Shenandoah University
e-mail account.
If for some reason you have a hold on your account, and you have not
received e-mail notification, please contact the wellness center
immediately to ascertain what additional documentation is needed.
