All applicants must submit the following to the Shenandoah University Office of Graduate Admissions:

- A completed application for admission and $30 application fee
- Not Required: GRE scores, recommendation forms, documentation of clinical hours, essay
- Unofficial transcripts uploaded from all higher education institutions attended (even if degree not awarded) for initial admissions.
- An uploaded copy of your current US physical therapy license
- Verification of current employment for at least 1 year as a physical therapist.
- For non-native English speakers licensed in one of the few U.S. states that does not require a TOEFL test for licensure, the applicant must demonstrate proficiency in the English language when applying to Shenandoah University. This program requires submission of the TOEFL test and a minimum score of 83 from applicants in this category.

- Applicants licensed in the following states must submit TOEFL scores:
- Colorado, Connecticut, Delaware, Florida, Indiana, Montana, New York, Pennsylvania, Wisconsin, and Puerto Rico

- All transcripts from non-U.S. based institutions must be course-by-course evaluated by a NACES accredited academic credential evaluation company to obtain equivalency information on course, grades, and degree earned. An uploaded copy of this course-by-course evaluation can serve as the unofficial transcript from that institution
- An official transcript from this institution, as well as an official copy of the evaluation, will be required if the applicant is admitted.

All materials should be uploaded directly to the application form. Do not mail paper copies of your transcripts before receiving an offer of admission. If admitted, applicants will be asked to request an official transcript from each institution from which they previously supplied unofficial transcripts.
