Students are required to have a completed health form on file at the Wilkins Wellness Center prior to the start of classes.
All students enrolled at Shenandoah University must complete the SU Health form and provide current insurance information.
In addition, the student must provide the following:
- Physical examination
- Two MMR (measles, mumps & rubella) vaccinations or a positive titer (blood test to prove immunity)
- Last date of the polio series or a positive titer
- Adult Tdap (tetanus, diphtheria & pertussis) within the past 10 years regardless of last Tdap vaccination
- Meningitis ACYW135 within the past 5 years or exemption form
- Two meningitis B vaccinations (dates depends on manufacturer) or exemption form
- Three hepatitis B vaccinations or a positive titer or exemption form
- Completion of the TB Assessment Form (page 5) of the health form OR a QuantiFERON TB Gold test within the last 12 months.
- A Positive TB skin test from an exposure, latent TB or BCG vaccination requires that the student complete QuantiFERON TB Gold test or have a chest x-ray and Statement of Treatment form (included on page 6 of the health form) “within” the last 12 months and must be signed by a MD/DO, NP or PA. Lab report and/or radiological report and Statement of Treatment form must be submitted with the health form to the Wellness Center.
Students are required to have a physical examination by their primary care provider.
Note: Commuter students, those not enrolled in a health program, or participating in a sport, are not required to have a physical examination; however, they must complete the remaining portions of the health form.
All students enrolled in Health Professions or School of Pharmacy must complete the SU Health Form. Currently, students can submit their information online at su.studenthealthportal.com. You must fill in the dates of your vaccinations and upload the documentation. All students enrolled in Health Professions and Pharmacy (which includes programs in Nursing, Occupational Therapy, Music Therapy, Physical Therapy, Athletic Training, Physician Assistant and Pharmacy) MUST complete a health form and provide current insurance information by the online process listed below.
In addition, the student must provide the following:
- Physical examination

- Two Varicella (chicken pox) vaccinations or a positive titer

- Three hepatitis B vaccinations or a positive titer or exemption form
- Current influenza vaccination after August 1st for the current flu and academic year
- Two TB skin tests and two readings with the second test and reading 7-21 days from the first test OR QuantiFERON TB Gold test within the last 12 months
- A Positive TB skin test from an exposure, latent TB or BCG vaccination requires that the student complete a chest x-ray and Statement of Treatment form (included on page 5 of the health form) “within” the last 12 months and must be signed by a MD/DO, NP or PA. Radiological report and Statement of Treatment form must be submitted with the health form to the Wellness Center.
All students admitted to the School of Nursing, School of Pharmacy, School of Health Professions (Physician Assistant Studies, Physical Therapy, Occupational Therapy, and Athletic Training) (collectively, “Health Professions”) must provide evidence of specific immunizations prior to the applicable date set forth on the Health and Insurance Requirements for Health Professions Students form. Students who fail to provide these documents before the deadline will have a $30 late fee charged to their student account and may have their admission revoked, or be suspended or dismissed from SU, and will not be allowed to attend classes, laboratory sessions, or clinical rotations. As a result of public health considerations, students admitted to a Health Professions program are not eligible for a Religious Exemption to the immunization requirements.
INSURANCE:
All health profession students are required to have a physical examination by their primary care provider.
Health Professions and Pharmacy students have additional immunization requirements that must be met prior to starting classes.
All students are required to have health insurance. Students are required to register their insurance information online. You must upload a copy of your current insurance card both front and back at su.studenthealthportal.com.
Online/distant learning students are exempt. This is called a waiver of the student plan.
To waive the Student Health Insurance plan:
- Log on to https://rcmdstudentbenefits.com/Shenandoah/
- Click on “Student Waiver or Enroll”
- Select with the RED “I want to Waive” button. If waiving the insurance, you’ll need information found on your current health insurance ID card. Upon completing your online form, you will be asked to review the information provided and click “submit” to complete the process.
The deadline to complete this online process is August 1st.
Student health insurance is still the best option for college students. Even though some states give subsidies, the difference in the deductible alone makes student insurance the most affordable option. Medicaid programs only cover the student in their home state.
All students who are registered must be covered under a health insurance plan.
Online/distant learning students are exempt. If you want to purchase the student health plan, follow the instructions below.
To enroll in the Student Health Insurance plan:
- Log on to student health insurance
- Click on “Student Waiver or Enroll”
- To ENROLL in the plan click on the GREEN “I want to Enroll” and follow the prompts. Immediately upon submitting your Online Form you will receive a confirmation number.
The deadline to complete this online process is August 1st.
International Student insurance plans, those purchased outside of the United States, must meet the guidelines of the Affordable Care Act 2010 regulations to be an accepted plan.
- Please visit our insurance website at student health insurance and create a user name and password.
- Click waive the student health insurance button and input your health insurance information into the website.
FirstStudent will audit the insurance to see if the policy meets the guidelines. If the policy meets the guidelines, you are considered covered as long as your insurance is valid while enrolled at Shenandoah. If your plan does not meet the guidelines, you will have to purchase a plan that does meet the requirements or enroll in the student health insurance policy approved by Shenandoah University. Most travel insurance policies do not meet the guidelines.
In addition to the University’s Health and Insurance Requirement form all athletes must complete the Medical Participation Forms and submit it to the athletic department along with a copy of the physical examination page.
Students participating in sports are required to have a physical examination by their primary care provider.
You must notify the Athletic Trainer immediately if you have an injury while participating in sports. A completed claim form must be submitted within 90 days from the date of injury. You can visit the website at: www.eiia.org/shenandoah
Every full-time student who participates in Intercollegiate athletics is automatically enrolled in the Athletic Accident Plan. In the event of an accident, you must:
- Report the accident to your coach or athletic trainer immediately.
- File all charges with your primary insurance carrier first. If you are insured by an HMO/PPO, you must obtain pre-authorization for all services rendered or benefits will be reduced by 50%.
- If the other insurance does not pay the entire bill, secure a claim form and instructions from the Athletic Department, fill in information completely, have the attending Physician and supervising coach complete their portion of the form, attach all itemized medical and hospital bills and mail them to the claims administrator.
