**This form must be filled out if the student-athlete is taking prescription medication(s).
- Sickle Cell | Trait Form
**This form is mandated by Shenandoah University and the NCAA.
- ADHD | NCAA-Mandated Form
**This form must be filled out if the student-athlete is taking prescription medication for ADHD.
- University Wellness Center Health | Wellness Center Physical Form
**Physical and Sickle Cell forms need to be electronically uploaded via the portal.
Other Forms & Policies
All students admitted to the School of Nursing, School of Pharmacy, School of Health Professions (Physician Assistant Studies, Physical Therapy, Occupational Therapy, and Athletic Training) must provide evidence of specific immunizations prior to the applicable date set forth on the Health and Insurance Requirements for Health Professions Students form. Students who fail to provide these documents before the deadline may have their admission revoked, or be suspended or dismissed from SU, and will not be allowed to attend classes, laboratory sessions, or clinical rotations. As a result of public health considerations, students admitted to a Health Professions program are not eligible for a Religious Exemption to the immunization requirements.
A late $27.00 late fee will be charged to the student account for any health form that is incomplete or not submitted by August 1st for the fall semester, December 15th for the spring semester, and June 1st for the summer semester.
