Faculty may use the “New Faculty Profile Form” to submit information for a new profile, edits for an existing one, or to remove a profile. Please choose the profile option on the form below. Once we receive your form submission, members of the OMC web team will create/update your online profile as soon as possible.
NEW! For faculty updating existing profiles, there is now an option to request the deletion of information within your profile.
PHOTO: This form asks for a horizontal profile photo. We like to showcase individual personality feel free to upload a professional image that showcases your unique character.
